Cards are replaced according to our current Terms, Conditions, and Refund policies. Complete this section for all American Heart Association CPR and First Aid Certification cards. Please make sure your Name and Current Address is correct. Unless otherwise specificed, cards are mailed to either the account address or your current address USPS First Class Mail. Postage included in price

Replacement cards are only issued for students of this Training Center. If you attended a course not sponsored by DVA Ambulance, please contact that Training Center directly. Otherwise Orders submitted for other Training Centers will not be fulfilled and the fee will not be refunded. Verify before submitting your request that you were in fact a student of this Training Center.

Misspelled/Incorrect Card Information:

Use this form to request a replacement card for misspellings or incorrect information. Every attempt is made to ensure your card is printed correctly, however errors can occur. If you received a card with an error, please use this form, you will be required to enter your payment information. However if we determine the error was cause on our part, your fee will be refunded.
